Form to be printed out and signed by parent then submitted to the main office (Rm. 127) to opt out of CAP
EMERGENCY CONTACT CARD
**Required form to be completed by parent/guardian, print-out or obtain from the main office (Rm. 127) and return to the main office. This form must be physically archived to allow school know WHICH ADULTS are allowed to pick-up a student or release to, in the event of an emergency. If the name of the adult picking up the child is NOT present on the form, the school will not be able to release that student.
ETHNIC AND RACE IDENTIFICATION FORM
Federal law requires the New York City Department of Education to collect and record the ethnic identity and race of public school students. This information is used to determine funding for your school, among other things, and is kept secure and confidential.
FALCON HEALTH CENTER CONSENT - LINK FOR MORE INFO
**Required form to be completed by parent/guardian, print-out or obtain from the main office (Rm. 127) and mail or return to the main office. This form must be physically archived to inform school of the family’s consent for students use of the health centers services.
This form is intended to address the McKinney-Vento Act 42 U.S.C. 11435, and must be completed for each student. The information you provide is confidential. Your child will not be discriminated against based upon the information provided.
ONLINE SCHOOL LUNCH FORM
**Required link to school lunch application. Each student submission ensures proper funding for the school. Research also shows that receiving free or reduced-price school lunches reduces food insecurity, obesity rates, and poor health. A hard copy can be available in the main office.
ADMISSIONS & ATHLETIC MEDICAL FORM
**Required 3 page form to be completed by students pediatrician, print out or obtain from the main office (Rm. 127) and return to the main office. Page 3 will cover and confirm students' ability to participate in athletic activities as advised by the family's pediatrician.
ATHLETIC PARENTAL CONSENT
**Required form to be completed by students family, for any student interested in participating in athletic activities. Print-out or obtain from the main office (Rm. 127) and return to the main office. By signing and submitting this form, parents are attesting that they have read the Concussion Management Info sheet.
VIDEO CONSENT FORM
Consent form to photograph, film, or videotape a student for non-profit, educational use only. Form is limited to classroom spaces and does not include auditorium space. Form can be printed, signed and submit to the appropriate teacher or submit to the main office.
