FALCON HEALTH CENTER CONSENT - LINK FOR MORE INFO 

**Required form to be completed by parent/guardian, print-out or obtain from the main office (Rm. 127) and mail or return to the main office. This form must be physically archived to inform school of the family’s consent for students use of the health centers services.

​ATHLETIC PARENTAL CONSENT a.k.a PSAL Interscholastic Athletics Parental Consent Form

**Required form to be completed by students family, for any student interested in participating in athletic activities. Print-out or obtain from the main office (Rm. 127) and return to the main office. By signing and submitting this form, parents are attesting that they have read the Concussion Management Info sheet.​

ADMISSIONS & ATHLETIC MEDICAL FORM a.k.a PSAL Medical Form

**Required 3 page form to be completed by students pediatrician, print out or obtain from the main office (Rm. 127) and return to the main office. Page 3 will cover and confirm students' ability to participate in athletic activities as advised by the family's pediatrician.

ONLINE SCHOOL LUNCH FORM

**Required link to school lunch application. Each student submission ensures proper funding for the school. Research also shows that receiving free or reduced-price school lunches reduces food insecurity, obesity rates, and poor health. A hard copy can be available in the main office.

PSAL CONCUSSION MANAGEMENT FORM 

**Required information for all students and families to review .

CHILD - ADOLESCENT HEALTH EXAM FORM

**Required 1 page form to be completed for all students by family pediatrician or health provider. Students NOT participating in athletic activities can use this health form.
